First Blood pays on the opening kill. In MOBAs it is a scripted in-game reward, in shooters it is a single duel, and the two are not the same bet.
The first kill carries an in-game gold bonus, which is why invades and level-one skirmishes matter. Aggressive junglers and lane bullies are the signal for which side draws it.
In Dota 2 and League of Legends the first kill of the game carries a built-in bonus for the team that lands it, so both sides have a mechanical reason to hunt or bait it.
